River Valley Skilled Nursing and Therapy ranked as the second largest nursing facility in Custer County during the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reports.

The data indicates the center provided 100 beds, exceeding the county average by six as of the first quarter of 2026. During the same period, they cared for an average of 48.9 residents per day.

River Valley Skilled Nursing and Therapy earned a perfect overall rating of 5 out of 5 from CMS, outpacing Oklahoma’s statewide nursing home average of 2.7.

In comparison, Clinton Therapy & Living Center was the largest in Custer County with 101 beds, while Corn Heritage Village and Rehab of Weatherford placed third with 81 beds available.

River Valley Skilled Nursing and Therapy also maintained a record of zero fines or penalties as of the first quarter of 2026.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
