The center had 118 available beds—25 above the county average as of the first quarter of 2026—and about 114 residents on average during the period.
CMS awarded Wilkins Health & Rehabilitation Community an overall rating of 4 out of 5, surpassing the 2.7 statewide average for nursing homes in Oklahoma.
ELK Crossing ranked as the largest nursing home in Stephens County for the quarter, offering 120 beds. Gregston Nursing Home, Inc. held the third spot, with 96 available beds.
No fines or penalties were recorded for Wilkins Health & Rehabilitation Community during the first quarter of 2026.

U.S. Census Bureau estimates indicate the country’s population of older adults continues to rise as younger cohorts decline. From 2020 through 2024, Americans age 65 and older grew by 13%, while the under-18 population dropped by 1.7%. “Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
